Recreation Participation Requirements
Recreation Registration Form and $30.00 annual fee for participants who are 15 years of age and older. Annual recreation fees are a contribution to service costs and are billed in January for each calendar year.
Consent For Emergency Medical Treatment form is required for all participants who attend programs without an aide or caregiver present. A renewal form will be sent when the participant’s form needs to be updated. It is most important to advise the Recreation Department immediately of any changes in the participant’s dietary restrictions, physical condition, caregiver address, or emergency phone number for the safety and protection of the client. Do not send or drop off clients who are not currently registered for Arc Recreation to social club, dances or any other recreation event, without prior approval from the Recreation Department.
This program serves individuals with intellectual and other developmental disabilities. Our goal is to maintain consumer participation in their chosen programs; however, we reserve the right to suspend consumers from active participation in recreation programs if they present a hazard to themselves or others. In the event that a consumer’s behaviors or health problems constitute a hazard, the consumer will be formally suspended until it is determined that the danger no longer exists. Case managers and caregivers will be consulted regarding any such suspensions or reinstatements.
